INSURANCE

While our services are covered under most insurance plans, note that every insurance policy is different and not all policies include this benefit. BIG realizes dealing with the insurance details can often be confusing to sort out, that's why were here to help you navigate through this process. We will take care of verifying your benefits, obtaining authorizations if and when required as well as working with your care representative assigned to your child’s case, all in an effort to reduce stress of this process so you can focus on your child. All that’s necessary to get started is your insurance information and proof of diagnosis, and we'll handle the rest.

Please complete the Insurance Verification Form below. Please note that you will be required to submit a photocopy of the front and back of your insurance card, as well as your proof of diagnosis in order to submit. Once your benefits have been verified, our Insurance Coordinator will contact you to review with you your coverage, our program options, and to answer any financial or insurance related questions you may have.

BIG currently accepts most major commercial and Medicaid insurance plans.

ENROLLMENT

When you are ready to enroll your child in our program, please complete and return the Enrollment Application, and copies of recent assessments. Once we have received the enrollment application, we can schedule a tour of the clinic. At the end of the tour our intake coordinator is available to answer any questions you may have. 

ASSESSMENT

Once we have reviewed your completed Enrollment Application, we will contact you to schedule an initial assessment. We require that at least one parent attend the assessment with the child. 

At the end of the assessment, we will discuss your child's schedule and a possible start date. Your child's start date will depend on current staff availablity. In some cases, BIG may have to hire and credential line staff with the Louisiana Behavior Analyst Board.
